Abstract

Pregnancy and labour are still continued to be associated with enormous health risks particularly in developing countries. Annually, 515,000 mothers die from complication of pregnancy and child birth. It is a tragic fact that, nearly 99% of these deaths occur in developing countries. Among this, many of the pregnancy and labour related complications cannot be predicted early and significant proportion of mothers die within 24 hours after delivery, indicating the significance of appropriate care in delivery and postpartum. The Postpartum haemorrhage is one of the leading causes of maternal death worldwide. 75-90% of these haemorrhage result from uterine atony. it occurs in about 10.5% of births and accounts for over 130 000 maternal deaths annually in the period of 20 years from April 1982 to march 2002 reveals that it was a contributory cause of maternal mortality in 19.9% of cases. the majority of deaths, (65%) had occurred within 24 hours of admission and in 47.5 % of cases there was severe anemia on admission, 17.5% had died due to an atonic PPH, which was the largest categories, followed by ruptured uterus (15%), retained placenta (12.5%), uterine inversion is a rare but potentially life-threatening obstetrics emergency. the incidence of uterine inversion varies and range from 1:25000 to 1:20000. Active management of the third stage of labour (AMTSL) is a combination of actions performed during the third stage of labour to prevent PPH, the components of AMTSL are administration of a uterotonic drug within one minute after the baby is born, controlled cord traction (CCT) and uterine massage immediately after delivery of the placenta Based on an extensive review of the literature, the joint statements by the ICM/FIGO recommends that active management of third-stage of labour be offered to all women, because the presence of risk factors cannot be used to predict postpartum haemorrhage.
